By submitting materials, I hereby grant PBM Products and Sam’s Club permission to use my likeness, my first name and first initial of my last name, city where currently living and/or the likeness of my child in a photograph in any and all of its publications, including website entries.
I understand and agree that these materials will become the property of PBM Products and Sam’s Club and will not be returned.
I am over 18 years of age. I am the lawful guardian of the photographed child. I have read this authorization and I fully understand its contents.
